Ankle Osteoarthritis: Understanding Your Treatment Options

Ankle osteoarthritis (OA) is a painful condition caused by gradual wear and damage to the smooth cartilage that lines the ankle joint. As the cartilage becomes thinner, the bones begin to rub together, leading to pain, stiffness, swelling and difficulty walking.

Unlike hip and knee arthritis, ankle osteoarthritis is most commonly caused by previous injury or trauma. Old ankle fractures, severe sprains, sporting injuries and ligament damage can all increase the likelihood of developing arthritis later in life.

At Elite Joint Solutions, we understand that every patient is different. Rather than relying on one treatment alone, we combine:

  • Expert assessment

  • Hands-on physiotherapy

  • Chiropractic-style joint mobilisation (where appropriate)

  • Soft tissue massage

  • Exercise rehabilitation

  • Acupuncture

  • Private prescribing (when appropriate)

  • Steroid injections

  • Hyaluronic acid injections

  • High-quality ankle braces and insoles

This combined approach aims to reduce pain, improve mobility and help you return to the activities you enjoy.

Common Symptoms

People with ankle osteoarthritis often describe:

  • Pain when walking

  • Morning stiffness

  • Swelling around the ankle

  • Difficulty walking on uneven ground

  • Reduced movement

  • Clicking or grinding sensations

  • Pain after prolonged standing

  • Reduced balance and confidence

Many patients notice their symptoms fluctuate, with flare-ups occurring after increased activity.

Gentle Self-Treatment

The majority of patients can improve significantly without surgery.

1. Keep Moving

Although it may seem counterintuitive, gentle movement actually nourishes the remaining cartilage.

Examples include:

  • Walking within comfort

  • Cycling

  • Swimming

  • Cross trainer

  • Gentle ankle mobility exercises

Complete rest usually causes the joint to become even stiffer.

2. Simple Ankle Exercises

Ankle Alphabet

Sit comfortably and slowly "draw" the alphabet using your foot.

Repeat 1-2 times daily.

Benefits:

Calf Stretch

Stand facing a wall.

Keep the affected leg straight behind you.

Lean forwards until you feel a stretch.

Hold 30 seconds.

Repeat 3-5 times.

Flexible calf muscles reduce stress through the ankle joint.

Heel Raises

Stand holding onto a worktop.

Rise onto your toes.

Lower slowly.

Aim for:

  • 3 sets of 10

This strengthens the calf muscles which help absorb forces during walking.

Resistance Band Exercises

Use an exercise band to strengthen:

  • Upward movement

  • Downward movement

  • Inward movement

  • Outward movement

Stronger muscles help stabilise the ankle and reduce joint loading.

Ice or Heat?

Both can help depending on your symptoms.

Ice

Ideal when:

  • Swollen

  • Inflamed

  • Following increased activity

Apply for:

10-15 minutes

Heat

Best when:

  • Stiff

  • Aching

  • Tight muscles

Heat helps improve circulation and loosen surrounding muscles before activity.

Many patients benefit from using heat before exercise and ice afterwards.

Medication

Pain relief can help you remain active.

Depending upon your medical history this may include:

  • Topical anti-inflammatory gels

  • Ibuprofen

  • Naproxen

  • Simple painkillers

  • Prescription anti-inflammatory medication

Medication should always be discussed with your GP or prescribing clinician to ensure it is safe for you. Current UK guidance generally favours topical NSAIDs first where appropriate and emphasises using medicines alongside exercise rather than instead of it.

Insoles Can Make A Huge Difference

Many people overlook footwear.

Well-designed insoles can:

  • Reduce pressure through painful areas

  • Improve foot alignment

  • Improve walking efficiency

  • Reduce fatigue

  • Improve comfort

Depending upon your foot posture we may recommend:

  • Cushioned insoles

  • Arch support

  • Heel wedges

  • Custom orthotics (where appropriate)

Sometimes a small change underneath the foot produces a surprisingly large reduction in ankle pain.

  • vasylli insoles to help with foot pain
vasylli insoles to help with foot pain
  • orthotics to help with ankle pain

The Benefits of Ankle Braces

4

One of the simplest treatments we recommend is a high-quality ankle brace.

We are pleased to work alongside EOS Active, who supply premium Össur braces.

A brace can:

  • Improve confidence walking

  • Reduce excessive ankle movement

  • Support weakened ligaments

  • Reduce pain

  • Improve balance

  • Help during longer walks

  • Allow many people to remain active

We particularly like Össur braces because they are:

  • Lightweight

  • Breathable

  • Comfortable

  • Durable

  • Easy to fit inside everyday footwear

Many patients find they can immediately walk further with less discomfort.

Hands-On Physiotherapy at Elite Joint Solutions

One thing that makes us different is our combined treatment approach.

Rather than simply prescribing exercises, treatment may include:

  • Physiotherapy

  • Chiropractic-style joint mobilisation (where clinically appropriate)

  • Massage therapy

  • Muscle release techniques

  • Progressive strengthening

  • Balance rehabilitation

By restoring movement in the surrounding joints and reducing muscle tightness, many patients find walking becomes significantly easier.

Acupuncture

For persistent pain we frequently use acupuncture.

Acupuncture may help:

  • Reduce pain

  • Relax tight muscles

  • Improve blood flow

  • Reduce muscle spasm

  • Make exercise more comfortable

Many patients choose to combine acupuncture with physiotherapy for optimal results.

Steroid Injections

If pain remains severe despite conservative treatment, a corticosteroid injection may be considered.

Steroid injections aim to:

  • Reduce inflammation

  • Reduce swelling

  • Improve walking

  • Allow more effective rehabilitation

Most patients who respond notice improvement within several days.

The benefits often last weeks to a few months, although responses vary considerably between individuals. NICE recommends steroid injections as a short-term option to support rehabilitation when other treatments have not provided sufficient relief.

Hyaluronic Acid Injections

Hyaluronic acid acts like a natural lubricant inside the joint.

The aim is to:

  • Improve lubrication

  • Reduce friction

  • Improve shock absorption

  • Reduce pain

  • Improve mobility

At Elite Joint Solutions we work with several leading manufacturers including:

  • Ostenil

  • Biolevox

  • Cingal

Some patients experience several months of symptom improvement following treatment.

  • hyaluronic acid for arthritis
  • ostenil for ankle osteoarthritis

What does the research say?

The evidence for hyaluronic acid injections in ankle osteoarthritis is more limited than for knee osteoarthritis. Recent systematic reviews have found mixed results, with some studies showing symptom improvement while others found little difference compared with control treatments.

Our approach is therefore individualised. We discuss the current evidence, your symptoms, previous treatments and goals before deciding whether hyaluronic acid is a reasonable option for you.

When Should You Seek Help?

Arrange an assessment if:

  • Pain has lasted more than 6 weeks

  • Walking is becoming difficult

  • Pain wakes you at night

  • The ankle repeatedly swells

  • You are relying on painkillers

  • Previous treatment hasn't helped

Early management can often slow progression and help maintain an active lifestyle.
