Shoulder pain can be extremely painful and debilitating. The shoulder is a complex joint surrounded by many different structures, which can make diagnosing the true source of pain difficult.






Scans such as MRI or ultrasound (USS) can sometimes help, but they can be expensive and are not always 100% accurate. In many cases, scan findings do not always match the symptoms you are experiencing.

What Is Shoulder Impingement?

One of the most common causes of shoulder pain is a condition known as shoulder impingement. This is an umbrella term used to describe pain originating from the subacromial space.

Within the subacromial space are several important structures, including:

  • The rotator cuff tendons:

    • supraspinatus

    • infraspinatus

    • subscapularis

  • The subacromial bursa

  • The long head of the biceps tendon

  • The inferior aspect of the acromioclavicular joint (ACJ)

Shoulder impingement is often characterised by pain during certain movements, particularly:

  • Reaching activities

  • Lifting the arm above shoulder height

  • Overhead activities

In many cases, there is no specific injury that triggers the symptoms. Patients commonly report pain when lying on the affected side at night, which can lead to significant sleep disturbance.

Why Diagnosis Can Be Difficult

Identifying exactly which structure is causing the pain can be challenging, and it is not always clear from clinical examination alone.

Although MRI and ultrasound scans can provide useful information, they do not always identify the true pain source. Some patients may have abnormalities on scans without any symptoms, while others experience significant pain despite relatively normal imaging findings.
