What is Atogepant?
Atogepant belongs to a new generation of migraine medications called CGRP receptor antagonists, often referred to as "gepants."
Unlike many older migraine medications that were originally designed for completely different conditions, Atogepant was developed specifically to prevent migraine.
It works by blocking the activity of Calcitonin Gene-Related Peptide (CGRP).
CGRP is a naturally occurring protein that plays a major role in migraine by:
Dilating blood vessels around the brain
Triggering inflammation around pain-sensitive nerves
Increasing pain transmission within the trigeminal nervous system
By blocking CGRP receptors, Atogepant reduces the cascade of events that leads to migraine attacks.
Rather than simply masking pain, it helps prevent migraines from developing in the first place.
How Effective is Atogepant?
Clinical trials have shown impressive reductions in the number of monthly migraine days in people with episodic migraine.

How Does Atogepant Compare with Older Migraine Medications?
Before CGRP-targeted medications became available, migraine prevention commonly relied on drugs originally developed for other medical conditions.

Amitriptyline
Originally developed as an antidepressant.

Weight gain

Topiramate
Topiramate is one of the most effective traditional migraine preventatives but is also associated with some of the highest discontinuation rates because of side effects.

Why Atogepant is Different
Rather than affecting multiple systems throughout the body, Atogepant specifically targets the CGRP pathway involved in migraine.
This targeted mechanism means many patients experience good migraine control with a favourable tolerability profile, although side effects such as constipation and nausea can still occur.
Is Atogepant Better Than Older Treatments?
There is currently no definitive head-to-head evidence proving Atogepant is superior to every older preventative medication, but studies suggest that CGRP-targeted treatments (including gepants) generally combine strong effectiveness with better tolerability than many traditional preventatives. Because of this, they are becoming an increasingly important option for patients whose previous treatments have failed or caused troublesome side effects.
Can Atogepant Treat an Acute Migraine?
This is an important distinction.
No. Atogepant is a preventative medication.
It is taken every day to reduce the number of migraine attacks over time.
If you develop a migraine despite taking Atogepant, you may still require an acute treatment such as:
Triptans
Anti-inflammatory medication
Anti-sickness medication
Other newer migraine medicines where appropriate
Your clinician can advise which combination is most suitable.
Is Atogepant Available on the NHS?
NICE recommends Atogepant for certain adults who experience frequent migraines, typically after at least three other preventative treatments have not worked, have not been tolerated, or are unsuitable. Eligibility depends on your individual clinical circumstances.
Private treatment may also be appropriate in selected cases following specialist assessment.
